Commission Request Form

       

There is a $50.00 Design/ Service Fee that must be upon approval of request. This will  cover the time, effort and communication necessary to create a customized pattern just for your costume. This fee is refundable only upon completion of your order. If you are not serious about purchasing a custom made costume please do not submit the form. Costume Prices will vary due to complexity of the design, materials involved and estimated time for completion.

 
FoxMoon Productions has the resources to find just about anything you might need for your costume. We will work diligently to assemble and/or create the perfect costume for you. We will sew what needs sewing, create what needs creating and find items that need to be found in order to make your costume experience the best it can be.  

Filling out this form does not mean you have placed an order. This is merely a request form to let us know you are interested so that we can start working on creating a pattern for your custom costume.

This is just for rough estimate of size. Measurements will be requested upon acceptance of your commission.

Upon Acceptance:

I understand that this is a commission request and agree to supply all information necessary should the request be accepted. I understand that this is a custom made product made to the measurements I will provide, due to this FoxMoon Productions is not responsible for miss-fitting items.

 There are three fees involved:

1.The Service Fee of $50.00 to start discussing the project and look into creating the patterns. After we have reviewed the information above we will contact you with further information.

2.The Product Fee may vary based upon item needed. If accepted we will send you a form to fill out with all required measurements and information for our files. Then we will give you an estimated production time and will then remain in contact with you as much as possible throughout the process of creating your product. At this time either full payment or a down payment will be needed in order to start. Payments must be completed before the product will ship out. 

3. Shipping will be based off of weight and location and therefore estimated after the product is completed.
